1. Amina has been diagnosed with PTSD. She experiences constant emotional trauma due to her kids living out of state. She misses them, and worries a lot about their safety.When she worries she will re-organize her home or compulsively clean to get her mind off of her worry thoughts. She does not recognize this behavioral/emotional pattern so this is something ARMHS has been providing education about. Amina also struggles with recurring nightmares, chronic pain, headaches and obesity.She can be somewhat narcissistic and overly confident as a way to compensate for low self-esteem. Client is satisfied with ARMHS services and is not interested in any other mental services at this time.Client MH symptoms presently do not interfere with her ability to access services. 3. Client is presently engaged in Adult Day Care but expresses having interest in helping out at a local homeless shelter. Client wants to volunteer but would only be able to do so when the weather is good as in winter it’s hard for her to move around and get outside. Client’s chronic pain, not being able to read, and memory problems make it very hard for her to get involved in the community. Due to mental health symptoms, she presently suffers fatigue and worry when in unfamiliar situations. There have been no significant changes in this area. 4. Client has eighth grade education from Somalia. She is enrolled in ESL and is learning English. Client has obstacles to functioning in certain environments due to her mental health symptoms and inability to read, write and speak English. 5. Client is sociable and enjoys the company of others. Client feels she’s familiar with what she thinks others do not know; meaning that she appears overly confident and lets others know how much she knows as a way to compensate for low self-esteem. Client attends Adult Day Care 2x/week where she meets and interacts with people of different tribes from her culture. Client does interact with family members in Africa-Somalia and USA.She’s does not feel comfortable and refuses to interact with others whom she does not know outside of her familiar environment/community (her apartment building and immediate neighborhood).

Client has been more consistent in keeping appointments and doing activities. She is still hesitant to engage with others outside of her community.As she is learning new skills, she has been observed to “show off” her skills and tends to dominate conversations. 6. Client reports having good relationships with family members in Africa and America. Client interacts with people in her Adult Daycare and her apartment building. Client interacts with her children when they are not busy. Client’ s symptoms normally do not interfere with her ability to associate and interact appropriately with others she is familiar with and has an established relationship 7. Client is able to perform her personal cares and grooming independently. She also cares for her apartment without assistance. When she worries or feels anxious, she tends to clean compulsively or re-arrange her house as a way to cope; in this way, her mental health symptoms interfere with her living capacity when neglected. 8.Amina reports that she has diabetes, high cholesterol, gas, left leg broken in the past and chronic pain.She used to have a seizure disorder that she now reports has resolved itself about 2 years ago.Client is unable to stand and pray but attends physical therapy to improve mobility. She would like to continue working on improving her health by consistently eating healthier and exercising regularly so she can lose more weight.Her mental health symptoms do not interfere with her ability to maintain her health.There have been no significant changes in this area. 9. Client lives on fixed income from SSI monthly to take care of her needs. Client is able to manage her financial needs independently; she has her own checking account at Wells Fargo and is able to pay her bills. Her mental health symptoms do not stand in the way of her ability to control her financial needs. There have been no significant changes in this area. 10. Amina is struggling with lots of memories , as well as her mental and physical health issues. She witnessed the civil war in Somalia and saw how family were killed and how people vanished with out a trace . She experienced broken leg during the war ; it was then she felt worthless , sad, no direction and hopelessness . Arnina ‘s social and interpersonal functions has caused her to want to be in charge . She struggles with low self-esteem when people are around her, like in her apartment she behaves as though she is the one in charge . Client said she struggles with persistent headaches , nightmares , flashbacks and at times no motivation. Amina is making some progress for the past six months since she start with ARMHS staff in being more independent doing her daily AOL ‘s. Amina is active with those she come close to in her community for social activities . Amina is making some progress in her memory , weight and her self-esteem . Amina is living with in her budget.
