Interview a friend or relative over the age of 45 using the Risk Stratification Assessment in the ACSM manual in Appendix B on page 159. Review the
questionnaire with the individual.

Part 2 – Written Report:

Determine the ACSM risk stratification classification for this person. Explain why you selected the ACSM risk stratification classification for this
person. Include your recommendation for this person regarding the next step in the HRPF exercise testing process. When writing your report, remember to
leave out the person’s real name. You may use a fictitious name if you desire. Include the completed Risk Stratification Form.

Your assignment should follow APA guidelines, and should include a minimum of three references.
