For example= respect patients autonomy- respect wishes and views if refusing treatment. However, unconscious patient, paramedics have to act in the best interest of the patient. HCPC guidelines for paramedics on unconscious patients.

Legal issues- DNR is a legal document ect

A call was dispatched to an 87 year old male complaining of SOB (Shortness of breath). The daughter of the patient phoned 999 because she was concerned for his welfare. On arrival the patient stated he did not want any help from the crew despite being noticeably SOB and very distressed. Before any history could be obtained the patient collapsed to the floor, he was unresponsive and not breathing.

The crew began CPR, the defibrillator pads were attached and the patient was intubated. No shocks were delivered as the patient’s heart rhythm was PEA, although the necessary drugs were administered. Suddenly the daughter became very distressed and stated the crew must stop as the patient had a DNR, at this point ALS (advance life support) had been in process for approximately 20 minutes. The daughter stated the patient had signed a DNR two days prior however it had not yet arrived at the house because it was still in the post.  The crew contacted the clinical support desk for advice whilst ALS continued, the crew were advised to contact the patient’s GP. The GP was contacted who confirmed the signed DNR and with the approval and authorization of the clinical support desk ALS was terminated. All equipment were detached and taken off and the patient was made comfortable.

 

 

 

The ethical and legal issues:

*autonomy, must respect patient’s wishes and views- if refusing treatment.

*unconscious patient has no capacity, cant gain consent. Always act in the best interest of the patient. HCPC regulations on unconscious patients.

*DNR- legal document .. paramedic must have proof of DNR-
