TITLE: Call Documentation – General Guidelines

PURPOSE: To document all calls to Nurse on Call using the Cleveland Clinic Protocols into Epic software program. All calls are audio-taped for quality improvement and training purposes.
PROCEDURES:
- The nurse must enter the Epic program at the beginning of a call. All documentation is completed on-line within the software program.
- The nurse is required to follow the call flow process in triaging symptom based calls and document accordingly. All interim care measures, medication recommendations and verbal orders are to be documented at the time of the call.
- For Health Information calls, the nurse is required to document reference materials in Epic
- The nurse must document the recommendation, callers understanding of recommendation and callers plan of action.
- Verbal and audio documentation is reviewed by peers and management for adherence to policy.
